Physician Consultation
As necessary, we consult with a resident’s physician
to communicate progress and provide customized treatments.
Insurance Billing
We accept payment from most insurances and Medicaid/Medicare,
as well as major credit cards.
A Continuum of Care
The Colonial Vista campus also includes retirement and assisted
living apartments, a home-health agency, and a subacute rehab
and outpatient adult and children’s therapy—so
people of all ages can find care that meets their changing
needs, in one convenient location.

Managing Your Care
The responsibility for your care lies
with you and your family—or others important to you—as
well as with your physician and other members of our interdisciplinary
team.
All will work together in developing an
appropriate plan of care. Our healthcare team includes:
A Warm and Friendly Atmosphere
The focus of your care while at Colonial
Vista Long-term Care Center will differ from your care in
the hospital, in a lesser care facility, or at home. Our goal
is to provide a home for adult living where the atmosphere
is warm and friendly and where residents feel safe and secure.
Team members will work with you on relearning and practicing
daily living skills such as dressing, bathing, walking, and
taking medications.
Most rooms are shared between two people.
Each personal space features a hospital bed and nightstand
for belongings. A closet is provided for clothing, and each
room offers a toilet and sink, with bathing and showers provided
in a supervised area for safety.
