home. Sometimes our ability to enjoy living at home can be
threatened by illness, accident, injury, or a long term disabling
condition.
Colonial Vista’s Healthy Options
Home Health and Chore Services is here to provide quality
Registered Nurses, Certified Nursing Assistants, Physical
Therapists, and other caregivers to help you remain in the
comfort of your own home.
Qualification
If your physician determines you need skilled care, and you
live in Chelan or Douglas County, you may qualify for home
health care. You or someone you know must be able to take
responsibility for your care when the home health staff is
not in your home.
Insurance Billing
The charge per visit will depend on the type of services you
receive. Home health services can be paid for in a number
of ways. Options include insurance, Medicare, Medicaid, other
third party payers, or private pay. We will help verify coverage
before starting care and directly bill your insurance for
our services. You will be informed of costs not covered by
your insurance company.
A Continuum of Care
The Colonial Vista campus includes retirement and assisted
living apartments, a long-term care facility, a subacute rehab
and therapy rehab center, and children's therapy—so
people of all ages can find care that meets their changing
needs, in one convenient location.

Services For All Your Home Care Needs
Our home health care professionals have
special training in home care. They can provide a wide range
of skilled services on a part-time or intermittent basis as
approved by your physician. We offer a variety of programs
to meet your needs.
Medicare Certified Home Health
These are physician-ordered skilled intermittent home health
services prescribed by your doctor, and require care from
an RN, physical therapist, or speech therapist to qualify.
Services also include care from an occupational therapist,
dietitian, social worker, and home health aide.
Private Duty Home Health Services
Supervised by your doctor, these services can include care
from an RN, therapist, social worker, or dietitian. Typically
these services are private-pay and can include lab draws,
medication set-up and evaluation, RN case management, maintenance
therapy programs, and nutritional programs.
Home and Personal Care Services
We provide nurse aides and caregivers to any person desiring
non-medical care such as assistance with personal care such
as bathing, grooming, dressing, feeding, exercise programs,
housekeeping, shopping, and meal planning and preparation.
Caregivers also provide respite, companion, and 24-hour care.
Medicare Certified Home Health Care
The Healthy Options staff are highly trained,
caring professionals who consistently go the extra mile to
assure that the needs of every individual is met. Our staff
participate in ongoing educational programs so that each staff
member is at the top of their field to provide you with the
best care available.

Nursing Care |
| Registered Nurses visit
your home to evaluate your condition and observe your
progress toward recovery. A registered nurse is available
24 hours a day, 7 days a week to coordinate needed care. |

Physical Therapy |
| Physical therapists help
you regain strength and mobility due to an illness, injury
or surgery affecting muscles, joints, tendons, or the
nervous system. |

Occupational Therapy |
| Our occupational therapists
can help you regain your ability to perform the activities
of daily living, or help you to adapt methods for accomplishing
these activities. |

Medical Social Work |
| Medical social workers
assist you and your family in coping with changing needs

emotional support and assistance in long term care planning

therapists teach techniques that may help with improvement. |
